not sure how much this is discussed in the book but the main 'cause' of adrenal fatigue is nutrient imbalance /inflammation toxin overload.
for me and my other health conditions - things like Zinc, magnesium, methylation cycle issues and excess of toxins are main issues for me and my adrenals.
Dr Lam writes good articles on this topic

Also Dr L Wilson (who is the guru on nutritional balancing especially minerals writes this

(I have his textbook and do quite a bit of reading of his articles regarding nutritional balancing)
Western A Price also has good article that mentions Adrenal Fatigue
These two problems—adrenal fatigue and toxic metal buildup—are strongly related because you cannot excrete metal without good adrenal function. Adrenal function needs to be pretty close to normal or you will start retaining metals, because the adrenal glands have such a big part to play in the proper handling of metals in the body.
The first sign of adrenal fatigue is reliance on stimulants. If the idea of going without your stimulant is an issue, you have adrenal fatigue. There, you have taken the test, now you know.

I read an article recently regarding "adrenal fatigue". I have translated some of it:
Adrenal fatigue is in fact a very rare condition, and a lot of nutrient imbalances/nutrient deficiencies which give exactly the same symptoms, are commonly defined as "adrenal fatigue", and so is the lack of energy production in the mitochondria of the cells (where ATP is produced), hormonal imbalances, reduced stomach acid production, inflammation of the gut, - which enhances abnormal serotonin activity, hypothyroidism, hypoglycemia, diabetes, allergies, food intolerances, chemical strains, frequent use of corn, soy, canola oil, plant oils, hydrogenated fats, gmo food, etc - will all give the symptoms which would be defined as adrenal fatigue or more correctly, adrenal insufficiency.
Besides meals with lots of proteins and fats, it was recommended to use topic magnesium, or magnesium baths (Epsom salt). Magnesium is the "antistress mineral number 1", as we have learnt. Also, eat every 3. hour to prevent blood sugar from decreasing.
A decrease in blood sugar promotes a stress reaction in our bodies. Each time the blood sugar falls under a certain level, the body will produce adrenalin and cortisol that lowers the metabolism (the energy production) and will release the glucose stored in the glycogen-stores in the liver and/or in the muscles, to use it as energy. Adrenalin also releases the stored fat as "fuel", which is okay in a healthy body, but not in a sick body, as these fatty acids stops the hormon production of the thyroid, which results in inflammation, which again will result in increased leves of serotonin, estrogen, even prolactin, more cortisol and adrenalin etc, and the cycle will continue for several hours, even after you have had yet another meal.
