Does that imply that improved medical services cause autoimmune diseases? 
Unfortunately, in a way, it does. IMO, the reason why they don't see it, is because in those countries with a lot of infectious diseases, people carry a heavy parasite load, so that autoimmune diseases are automatically prevented. Equally important - they don't seek medical help, unless they have a serious problem, because you don't walk 50 to a hundred miles or more, through lion/leopard country, to see a doctor, (whom you don't have the money to pay, anyway), unless you have a mighty good reason, and a case of hay fever, asthma, exzema, etc., is not a qualifying reason, obviously. When you live with parasites every day of your life, and worry constantly about catching aids, ebola hemorrhagic fever, lassa fever, malaria, etc., allergies aren't even on your radar. And, of course, most of them do get plenty of vitamin D from the sun.

Babies born in developing countries are delivered the old fashioned way, not by c-section. They are also breastfed. Studies show that babies get a healthy dose of good bacteria from vaginal deliveries and that c-section babies have more allergies and are prone to more infections, so maybe this does play into the hygiene theory a bit because their immune systems aren't developing as they should. Additionally babies in developed countries are also kept inside and when they are out they are covered in SPF 50, so they don't get adequate vitamin D from the sun. We load them up on casein, gluten, HFCS and chemicals and preservatives. Basically we are doing everything wrong and it all probably contributes to what we are seeing.

I certainly wouldn't argue with that. Everything has an effect. As a society, we do a lot of things, that individually, aren't a big deal. Unfortunately, though, when you start adding them all together, some combinations can have synergistic effects, and the cumulative effect can quickly become a big deal. With each advancement in medical science, we move another step farther away from our roots. It does seem rather inexplicable, that we continue to "advance" our "understanding" of ways to improve our health, by moving in the wrong direction.
We continue to think that we know better than Mother Nature, despite the fact that we continue to prove that we do not.
As a society, it seems that the more advanced we become, the more dysfunctional we become.

children in developing countries eat produce that they grow and cook it using traditional cooking methods
I worked with a lady who was born and grew up in the remote highlands of Papua New Guinea. She grew up eating Banana's, yam (sweet potato), rice, leafy greens and pork. (and any berries or nuts that grew nearby)
Chicken was a once every 3 month or 6 months treat (i chicken shared amongst the large family so she got like a tablespoon of meat).
She did not realise that other fruits actually existed until she was 18 and went to uni in the capital city
She did not know that beef or lamb existed or even tried it until she came to Australia when she was about 21.
