*How to Participate: *To receive a complimentary one-hour consultation with
an ADA member Registered Dietitian, visit www.themoderationnation.com and
follow the directions provided to print out a participation form (hereafter,
the “Certificate”), while supplies last, then take the following steps:

1) Contact a participating ADA member Registered Dietitian in your area
to schedule a consultation. A listing of ADA member Registered Dietitians is
available at www.themoderationnation.com. *The consultation must take place
within forty-five (45) days of the date printed on your Certificate or the
offer will be void. *Be sure to confirm that the Registered Dietitian will
accept your Certificate prior to your consultation. If the Registered
Dietitian (as defined below) does not accept the Certificate in lieu of
payment, you may choose to pay the consultation fees at the time of your
visit and submit the Certificate by mail to request reimbursement up to
$250.00;

2) Complete the Certificate and print it to bring to your consultation
with the chosen Registered Dietitian within forty-five (45) days of printing
the Certificate;

3) Upon completing your consultation, you and the Registered Dietitian
must sign and date your Certificate in each other’s presence; in addition,
the Registered Dietitian will ask you for a valid form of I.D. such as a
driver’s license, to verify your eligibility;

4) Following signature and verification, if the Registered Dietitian
accepts Certificates in lieu of payment, submit your Certificate to your
Registered Dietitian, who will complete any applicable fields required and
submit the Certificate to HCHN.

5) If the Registered Dietitian does not accept Certificates in lieu
of payment, you must pay all consultation fees at the time of visit and mail
the completed Certificate, along with a copy of your driver’s license and a
copy of the consultation receipt with the Certificate, to Hershey Moderation
Nation Offer P.O. Box 3040, Young America, MN 55558, to request
reimbursement for the consultation fee up to $250.00. Please ensure that any
personal medical information and/or medical billing codes are excluded from
the receipt.

(Collectively “Offer”). Certificate must be postmarked within thirty (30)
days of the date of the consultation as stated on the consultation invoice
and in no event received later than March 16, 2011 or it will be void and no
reimbursement will be provided for the consultation at issue. HCHN is not
responsible for reimbursement for any late Certificates, regardless of the
cause, including lateness which results from the ADA member Registered
Dietitian or other third party delay. Any difference between the value of
the Certificate and the actual cost of the consultation will not be
reimbursed. All submitted materials become the property of HCHN and will not
be returned or acknowledged.

*Registered Dietitian Eligibility: *This Offer is only valid for
participating Registered Dietitians who (i) are legal residents of the 50
United States or District of Columbia of legal age of majority in their
jurisdiction of residence (and at least 18 years of age) and; (ii) are an
active ADA member Registered Dietitian in good standing. It is the
participant’s responsibility to confirm that the Registered Dietitian is
eligible and an active American Dietetic Association member Registered
Dietitian.

*Consultation Conditions: Your consultation must be redeemed within
forty-five (45) days of the date printed on your Certificate, after which
the Certificate will be void and no reimbursement will be made. *Certificate
is valid only for one (1) consultation that is *up to one (1) hour *in
length and valued up to $250.00. Should any consultation exceed one (1) hour
or $250.00, participants will be responsible for paying any fees due for any
amount of time exceeding $250.00. Consultations may not be aggregated to
meet the one (1) hour criteria. *Limit one (1) consultation and Certificate
per person/email address. *Any Certificate received from any person in
excess of the stated limitations will be void and the participant who
submitted such Certificate will not be reimbursed and the participant may be
responsible for the full consultation fee charged by the Registered
Dietitian. *Participants will be responsible for travel to/from any ADA
member Registered Dietitian’s office. *Consultations may be subject to
additional terms and conditions specified by each applicable Registered
Dietitian and will depend on each Registered Dietitian’s patient policy.
.Consultation appointments are subject to availability and the Registered
Dietitians’ schedules. The decisions of HCHN with respect to all
Certificates are final and binding. HCHN is not responsible for any fees
that result from a consultation with a Registered Dietitian who is not
participating in this offer.

*Certificate Verification: *A Certificate may only be used once. Each
Certificate printed from the website www.themoderationnation.com will
contain a unique Certificate Number. Certificates will be void if determined
by HCHN in its sole discretion to be unauthorized or illegitimately obtained
or tampered with. Certificates that contain printing, mechanical or other
errors, that are photocopied, printed or produced in error, that are
unreadable or blank, that are not verified or recognized as being validly
issued by HCHN in this offer, that are determined to have been previously
entered and used, and that are incorrectly or incompletely submitted will be
void. All redemptions made with such Certificates are void and the
participant who submitted such Certificates may be responsible for paying
any fees due to the corresponding Registered Dietitian. All Certificates
submitted become property of HCHN and will not be returned. Please allow 6-8
weeks for receipt of reimbursement.

*Registered Dietitian Procedures: *In the event participating ADA member
Registered Dietician will accept the Certificate in lieu of payment, the
following steps must occur for reimbursement of costs associated with the
office visit. *PLEASE NOTE: Consultation must take place within forty-five
days of the date printed on the Certificate presented by patient. *

1) Confirm the consultation has taken place within forty-five (45) days
of the date printed on the Certificate submitted by participant. If the
consultation has not taken place within forty-five days, certificate is void
and payment will not be provided;

2) Confirm the Certificate has been completed with all the required
information;

3) Upon completing your consultation, you and the participant must sign

Not all dietitians are up to speed on MC, but I know some are . . . be sure and tell them about Polly's success with diet and what is said on this site.

I have an appointment on Monday. I dropped off a few things to her last night. I'll take a food diary (2 days) and my vitamins when I see her.

MB, what is the price range for an hourly visit with a dietician? I'm sure it varies geographically etc.

Just guessing, but probably anywhere from $80-150 depending on the location.

Thanks, just curious. I asked her but she just said is wasn't anything like $250.

BTW, she said that she did vaguely remember something about the program in a journal.

She recommended L-Glutamine 500 mg and L-Arginine 500 mg (Spring Valley Brand is what she takes. Says it is soothing. She had bowel cancer.)

She was really concerned that is wasn't on cholesterol/tryglycerides meds as am I.........

I must excercise 20 minutes a day.

I need to add calories to my daily intake.

She looked over my supplements. Warned about the danger of too much Vitamin D.
She spent about 1 and 1/2 hours with me.

Hi Mary Beth, I just wanted to let you know that I booked my appointment with my dietitian Marilyn Mills today for October 15th using that voucher. Her office will accept the voucher and submit it directly.
